An article in the Society for Healthcare Epidemiology of America (SHEA) in March 2014 discussed the prevention of central line-associated bloodstream infection (CLABSI) among pediatric oncology patients. Studies suggest that ambulatory (outpatient) patients experience 2-3 times as many CLABSIs as hospitalized patients. The investigators focused their efforts on the 3 groups who access the central lines of ambulatory pediatric oncology patients most commonly: Clinic nurses, Home Care nurses and Patients’ families. The goal was to enlist Families and Homecare Agencies to Prevent CLABSI.
Over a 24-month program CLABSI rates decreased by 48%. The investigators calculated that they may have prevented 73 hospital admissions for bacteremia, prevented 33 CLABSIs, and saved almost $1.5 million.
What should you take away from this? Engaging clinic nurses, families, and home care agencies in central-line maintenance can have a significant impact on bacteremia and CLABSI in pediatric oncology patients. These encouraging results should prompt additional research into central-line maintenance for prevention of CLABSI and bacteremia in adult oncology patients as well as other pediatric and adult populations with central lines.
